Long Term Health Support Services

W2W is a provider of specialist vocational rehabilitation and support services. W2W helps employees with illness and long-term conditions to self-manage their condition to remain active and to return to normal daily activities including work.

 

All W2W clinical health coaches are registered health professionals with many years experience. They educate patients and offer support, advice and guidance to facilitate rehabilitation and sustainable return to work.

 

Clinical Health Coaches will also work closely with employers to facilitate return to work through return to work planning, advice, recommendation of reasonable adjustments and advice on longer term management.

 

This model is applied to all W2W Work Support Services including:

• Covid 19 Work Support Service

Cancer Work Support Service 

Our Cancer Work Support Service has been described by our patients as a ‘lifeline’ during their recovery from cancer and associated treatments.

 

We help people at any stage during their cancer experience to help them to come to terms with their illness and to self-manage their symptoms to improve their quality of life and where possible to return to work.

 

Not only do we provide support for patients, we also help employers to provide the best support possible for their returning employee.

Chronic Illness Support Service 

Our Chronic Illness Work Support Service helps people with chronic health conditions, who would like support getting back to work or to maintain or improve their life at work.

 

This often involves helping them to manage symptoms such as pain, low mood and fatigue, We use an interdisciplinary approach through health coaching, and support from physiotherapists and psychological therapy to meet individual needs.

 

We support people who have:
– Persistent pain
– Chronic fatigue
– Mental ill-health (mild to moderate)
– Neurological conditions (including Stroke, Parkinson’s Disease and MS)

At at the heart of our support services is functional restoration and return to work focus.

Remote Functional Restoration Programme (FRP) 

W2W provide a remote-based Functional Rehabilitation Programme (FRP) which complements our Chronic Illness Work Support Service

 

Our FRP provides in-depth functional assessment and tailored rehabilitation exercise which support employees to return to work and improved quality of life.

Covid-19 Work Support Service 

W2W provide a work support service for those experiencing the symptoms of Long Covid. The service is designed to support people to self-manage their symptoms and to support them with their return to work plans when ready.

 

There is a strong focus on adapting to a new illness that has significant symptoms affecting quality of life. Support is provided to address any psychological difficulties including fear of fatigue, frustration and low mood, physical health needs such as managing deconditioning and increasing stamina and vocational rehabilitation including employer engagement support.

Fitness for Work Assessment (FFWA) and Functional Capacity Evaluation (FCE)

Fitness for work assessments are a comprehensive evaluation of an individual in relation to health and work. The aim is to identify an employee’s: physical limitations, functional capacity concerning job demands, psychosocial barriers to recovery and return-to-work, following a period of absence.

 

This consists of:
• Subjective and objective musculoskeletal assessment
• Functional testing
• Psychosocial screening

 

A Functional Capacity Evaluation is the functional testing component of a Fitness for work assessment and can be carried out in isolation to identify functional limitations in relation to job demands.

 

Both assessments are carried out by a W2W Occupational Health Physiotherapist. The aim of these assessments is to ensure appropriate work adjustments are made to allow an individual to engage/maintain work ability and to inform ongoing functional rehabilitation programmes, to facilitate sustainable return to normal work.
